As the parent(s)/guardian(s) of the registered child, a minor, we authorize Little Steamers Nursery to consent to any diagnostic procedure or medical care deemed advisable by a licensed physician or surgeon.

This authorization is given in advance for any required treatment and grants authority to our agent(s) to consent to any diagnosis, treatment, or hospital care a physician deems necessary. Parents are solely responsible for all medical and transport expenses to the medical facility. This authorization remains effective until the child is withdrawn from Little Steamers Nursery or revoked in writing and delivered to the agent(s).

While the nursery is child-proofed and children are well supervised, accidents may occur. We, the undersigned, assume all risks of injury or harm to our child associated with their participation in the nursery. We release, indemnify, defend, and discharge Little Steamers Nursery and its staff, employees, and agents from all liability, claims, demands, damages, costs, expenses, actions, and causes of action regarding injury, death, loss, or damage to the child or caused by the child, arising during the child's participation in the nursery. We also waive and release any rights and claims for damages against Little Steamers Nursery and its representatives for any injuries suffered by our child during drop-off and pick-up.

I hereby grant and authorize undefined the right to take, edit, copy, publish, distribute and make use of any and all pictures or video taken of my child(ren) to be used in and/or for legally promotional materials and digital communications. This authorization shall continue indefinitely, unless I otherwise revoke said authorization in writing. I understand and agree that these materials shall become the property of Little Steamers Nursery and will not be returned.

1. Fees are payable monthly, regardless of vacations, absences, or decisions to keep a child at home during Ramadan. Failure to pay fees for any given month will result in the child's removal from the Nursery's register.
2. Monthly fees are due either by the end of each month or no later than the first day of the following month. 
3. If payment is not received by the first day of the month, a reminder will be sent to the parents.
4. A late payment fee of QR 500 will be applied if fees are not paid by the 3rd business day of the month.
5. A service charge of QR 300 will be incurred for returned cheques due to insufficient funds or incorrect signatures.
6. If fees remain unpaid by the end of the first week of the month, the child will be removed from the Nursery. After withdrawal, requests for certificates or assessments will no longer be processed.
7. One-month 50% fee discount: Parents may choose to apply a 50% discount on fees for one month per academic year, to be used during Ramadan or a vacation month within the academic year.
8. Fees are due from the date a place in the Nursery has been accepted and the start date confirmed, irrespective of any decision to delay the start date. If the start date is deferred for any reason after accepting a place, full fees are still required from the originally agreed-upon start date noted on the child's registration form.
    It is important for parents to be aware that further details pertaining to the fee policy and registration process can be found within the Little Steamers Nursery Policies and Procedures document. By signing this registration form, parents hereby confirm that they have thoroughly reviewed and understood the contents of the Policies and Procedures document and consent to adhere to its terms. Should any concerns or objections arise, parents are obliged to submit their concerns in writing to the management for further consideration.

By enrolling your child into Little Steamers Nursery, you acknowledge that you have read, understood and agreed to abide by all Little Steamers Nursery policies and Procedures and by all sections of this application. Little Steamers Nursery reserves the right to change its fees, terms, conditions and policies at any time and without prior notices.
